Smart medicine starts upstream

Looking for Resources to Help Patients in Need? Meet HB-Mapper

HB-Mapper is an easy-to-use, web platform that allows clinics to find and share information with patients on available social services and resources in their communities. We work with community partners, including high school students, to research and add local resources to the HB-Mapper database while learning about an upstream approach to healthcare. We call this model 'HealthDetailing 2.0'. Want to learn more? Want HB-Mapper for your clinic? Sign up today and let us know!

Need Funds for a Health Project?     Check out HBFunder.org

Getting important health projects off the ground is hard. We built a crowdfunding platform to help community clinics and nonprofits raise funds for projects that address patients' social needs and improve social determinants of health. Unlike other crowdfunding sites, we reinvest a portion of net proceeds back into successful nonprofits, especially clinic-community partnerships that are improving health where it begins.

An Online WorkPlan for Social Determinants of Health

We know how to redesign healthcare to better care for patients and communities with complex social needs. We distilled that expertise into a simple cloud-based workplan to help clinics and community groups develop and improve interventions that tackle social determinants of health. Want to join our beta? "Rishi Manchanda and his colleagues at HealthBegins have the right mindset: a deep respect for the tools, new and less so, we need to take care of the sick and to prevent unnecessary suffering; a knowledge of our health care system and its weaknesses and assets; an awareness of the importance of civic engagement in addressing upstream and downstream problems; a good sense of the human resources we might need, upstreamist clinicians among them, to transform American health care delivery."    - Paul Farmer MD PhD (click to read more of Paul Farmer's essay on "The Upstream Doctors")
